Furious Stylez LLC Private Lesson Bundle Agreement
Agreement for One-Time Purchase of Four Private Lessons
This Private Lesson Bundle Contract (“Agreement”) is made between Furious Stylez LLC (“Business”) and the undersigned client (“Client”) for the purchase of a bundle of four private lessons. By continuing to make the purchase, the Client declares they read and agree to the following terms and conditions:
1. Bundle Details and Fees
Client is purchasing a one-time bundle of four private lessons for a total fee of $200, before eligible discounts. Lessons are limited to one per week and must be scheduled in advance with a Business instructor. All four lessons must be completed within two months of the purchase date. Unused lessons after this period will be forfeited and are non-refundable.
2. Scheduling and Attendance
Private lessons must be scheduled in advance and are subject to instructor availability. Cancellations or requests to reschedule require at least 24 hours’ advance notice. Failure to provide adequate notice may result in forfeiture of the scheduled lesson.
3. Code of Conduct
Client agrees to abide by all Business rules, regulations, and code of conduct. The Business reserves the right to suspend or terminate access to lessons for violations of Business policies or behavior deemed detrimental to other clients or staff.
4. Waiver and Release of Liability
Client acknowledges that participation in Jiu Jitsu involves physical activity and assumes all risks associated with training. Client releases Furious Stylez LLC, its owners, instructors, and staff from any liability for injuries or damages incurred during participation.
5. Miscellaneous
- This Agreement constitutes the entire understanding between the Business and Client regarding the private lesson bundle.
- Any modifications to this Agreement must be made in writing and signed by both parties.
- This Agreement is governed by the laws of the State of Texas.
By continuing on with the purchase of the Private Lesson Bundle, Client acknowledges and agrees to all terms and conditions outlined in this Agreement.
