Furious Stylez LLC Private Lessons Membership Contract
Membership Agreement for Private Lessons
This Membership Contract (“Agreement”) is entered into by and between Furious Stylez LLC (“Business”) and the undersigned member (“Member”) for the Private Lessons Membership. By continuing to make the purchase, the Member declares they read and agree to the following terms and conditions:
1. Membership and Fees
Member enrolls in the Private Lessons Membership at a rate of $160 per month, before any eligible discounts. The monthly fee is due on the first day of each month and covers up to one private lesson per week, scheduled with the Business’ instructors. The first month’s fee will be prorated based on the Member’s start date.
2. Term and Termination
This Agreement is effective on a month-to-month basis until terminated by either party. Member may cancel membership by providing written notice to the Business at least thirty (30) days prior to the intended cancellation date. Membership fees will continue to be charged through the notice period.
3. Late Payment Policy
If the monthly membership fee is not received within five (5) days of the due date, a late payment fee of $25 will be assessed. Continued failure to pay may result in suspension or termination of membership and forfeiture of access to private lessons.
4. Scheduling and Attendance
Private lessons are limited to one per week, must be scheduled in advance, and are subject to instructor availability. Cancellations or rescheduling of lessons require at least 24 hours’ advance notice. Failure to provide adequate notice may result in forfeiture of the lesson.
5. Code of Conduct
Member agrees to abide by all Business rules, regulations, and code of conduct. The Business reserves the right to suspend or terminate membership for violations of Business policies or behavior deemed detrimental to other members or staff.
6. Waiver and Release of Liability
Member acknowledges that participation in Jiu Jitsu involves physical activity and assumes all risks associated with training. Member releases Furious Stylez LLC, its owners, instructors, and staff from any liability for injuries or damages incurred during participation.
7. Miscellaneous
- This Agreement constitutes the entire understanding between the Business and Member regarding the Private Lessons Membership.
- Any modifications to this Agreement must be made in writing and signed by both parties.
- This Agreement is governed by the laws of the State of Texas.
By continuing on to purchase the Private Lesson Membership, Member acknowledges and agrees to all terms and conditions outlined in this Agreement.
