Waiver

In consideration of being permitted to participate in any way in the Martial Arts Program, as defined below, by this business, FURIOUS STYLEZ LLC (the “Business”), I, or, if a minor, the parent(s) and/or legal guardian of the minor participant named below (hereinafter, collectively, “I,” “me” or “my”), enter into this Release, Liability Waiver and Indemnity Agreement (“Agreement”) and hereby agree:

Training & Media Release Waiver

As a member/participant of Furious Stylez LLC of Texas, I/my child, intend(s) to and will engage in strenuous physical activities and instruction on the premises selected or virtually. I understand that these activities involve certain risk and exposure to personal injury, to which I assume responsibility for myself and/or my child or any other family member who may participate. In consideration, I hereby release in full and forever Furious Stylez LLC owners, directors, shareholders, agents, trainers, and any employee, whether acting officially or otherwise on behalf of myself or family member or representative heirs, executors, administration, and personal representative from any gross negligence, sickness, claims, demands, or cause of action relating to or deriving from any injury to myself or family member including children during, or arising from training, guidance, recommendations, etc. of Furious Stylez LLC of Texas.

I also grant Furious Stylez LLC employees and representatives the right to take photographs of myself, child, or any accompanying individual, and property while participating in & receiving the services of Furious Stylez and to use and publish these photos or videos in print and/or electronically. I agree that Furious Stylez may use such media with or without my name and for any lawful purpose, including, for example, such purpose as publicity, illustration, advertisement, and web content. I understand and agree that Furious Stylez LLC reserves the right to terminate any membership and refuse service to anyone at any given time with or without consent of members.

Rules & Etiquette

• Maintain proper hygiene: Always wear a clean training attire to every private lesson, seminar, personal training session, etc. Attire must be washed after every use. Keep fingernails and toenails neatly trimmed. All cuts/abrasions must be cleaned and covered properly at all times.
• All jewelry, piercings, necklaces, and other items should be removed during training
• Flip Flops, slides, or Crocs are part of your uniform and they must be brought to every Jiu Jitsu lesson. You must always wear them when you are off the mats.
• No shoes, food, or drinks allowed on the mats
• For No Gi, you must wear a tight fitting shirt/rashguard and shorts or leggings with no pockets
• Take care of your training partner: no cranking for a submission that could injure your training partner. Always be aware of their tap whather it is physical or verbal.
• Parents absolutely NO coaching of your children or other students during the lesson – direct questions to us
• No talking while the instructor is demonstrating techniques
• Cancellations require a 30 day notification via the Request Cancellation button inside your BJJLINK profile
• Please reschedule if you are sick. Jiu Jitsu is an extremely close contact sport and you want to be respectful of your training partners and coaches, do not get them sick.
• At least a 24-hour notice, unless a sickness or injury popped up, to cancel a scheduled session. If 24-hour notice is not provided, then the private lesson will be forfeited, whether it was a single lesson, part of a 4-pack bundle (the forfeited lesson would count as one of the four lessons), or part of a Black level membership (the forfeited lesson would be that week’s lesson). You will be provided an option to reschedule for $30 so as not to forfeit the lesson.
• WE RESERVE THE RIGHT TO CANCEL YOUR MEMBERSHIP AT ANY TIME!

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY CONTINUING WITH THE PURCHASE THROUGH THIS WEBSITE, AND WILL DO SO FREELY AND VOLUNTARILY WITHOUT ANY COERCION, INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY PURCHASE TO ACT AS MY SIGNATURE AND TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY FROM THIS BUSINESS TO THE FULLEST EXTENT PERMITTED BY LAW.
