Furious Stylez LLC Private Lesson Agreement

Agreement for One-Time Purchase of a Single Private Lesson

This Private Lesson Contract (“Agreement”) is made between Furious Stylez LLC (“Business”) and the undersigned client (“Client”) for the purchase of a single private lesson. By continuing to make the purchase, the Client declares they read and agree to the following terms and conditions:

1. Lesson Details and Fees

Client is purchasing a one-time private lesson for a total fee of $60, before any applicable discounts. The lesson must be scheduled in advance with a Business instructor and completed within one month of the purchase date. If the lesson is not used within this period, it will be forfeited and is non-refundable.

2. Scheduling and Attendance

The private lesson must be scheduled in advance and is subject to instructor availability. Cancellations or requests to reschedule require at least 24 hours’ advance notice. Failure to provide adequate notice may result in forfeiture of the scheduled lesson.

3. Code of Conduct

Client agrees to abide by all Business rules, regulations, and code of conduct. The Business reserves the right to suspend or terminate access to lessons for violations of Business policies or behavior deemed detrimental to other clients or staff.

4. Waiver and Release of Liability

Client acknowledges that participation in Jiu Jitsu involves physical activity and assumes all risks associated with training. Client releases Furious Stylez LLC, its owners, instructors, and staff from any liability for injuries or damages incurred during participation.

5. Miscellaneous

• This Agreement constitutes the entire understanding between the Business and Client regarding the private lesson purchase.
• Any modifications to this Agreement must be made in writing and signed by both parties.
• This Agreement is governed by the laws of the State of Texas.

By continuing on to make the purchase, Client acknowledges and agrees to all terms and conditions outlined in this Agreement.
